Wrinkles are badges of rank made throughout years of life. As much as people move and work, wrinkles leave marks on people wide and deep. They’re the strongest enemy to ladies who chase after young-looking face. Here we see new groups of consumers such as Nomujok (middle-aged men who pursue young and sophisticated look and lifestyle) and Urban granny (middle-aged people living in city who rather enjoy their life than looking after their grandchildren) appeared, which changed the question ‘can we remove wrinkles or not’ to ‘how can we remove them in an effective and natural way?’ We met Dr.SangHoon Park of ID Hospital and have searched to see if there’s any non-incisional surgery that can remove wrinkles naturally with an immediate effect.

There’s a new consumer group for getting plastic surgery?

Not long ago, most of the people who showed their interest in wrinkle removal procedure or surgery were middle-aged women. Today, however, things have changed a little bit. Professionals with high-income or high-degree, business women, CEOs or board members in their 40~60s now have a stronger desire to have a competitive advantage through young-looking face. They are leading a new trend so called ‘business plastic surgery’, actively taking the lead in removing wrinkles or getting plastic surgery. Especially since they are people who are currently in their field, they show a clear preference of non-incisional procedure or thread lifting for natural result without leaving scars to surgeries that cause problems in normal daily routine.

What kind of procedures are there for removing wrinkles?

There is filler, botox, and laser treatment for removing light wrinkles. For clear effect there is thread lifting and face lifting, which is a surgical method. Filler and botox are not effective for deep wrinkles, and their lasting effect is short. Laser treatment such as Ulthera and Thermage are inconvenient in a way that you have to go through multiple times of procedure. With face lifting you can definitely see its effect, but you might have scars left because of incisions made, and the recovery period is long. Thread lifting is an improved procedure that cover such flaws of face lifting surgery. Thread lifting, however, does not reach SMAS layer under the skin, so after a certain period of time the skin becomes saggy again. Non-incisional SMAS lifting, therefore, is an improved procedure that satisfies the needs of a new group of consumers.

Is SMAS layer important?

SMAS layer refers to Superficial Musculoaponeurotic System that is located between subcutaneous layer under dermis and muscular layer. Fibrous Membrane is entwined like a net in a space between the skin and the muscle. Once SMAS layer droops, which is sensitive to gravity, upper dermis and fat layer become saggy, and your skin loses elasticity, creating wrinkles. Existing thread lifting uses a non-incisional method, pulling subcutaneous layer outside the skin instead of SMAS layer, so you can only see limited lifting effect. SMAS lifting, however, pulls the layer in one time, so the lifting effect is much greater.

Does non-incisional SMAS lifting improve elasticity of SMAS layer?

Yes. Non-incisional SMAS lifting has combined the definite effect of face lifting with non-incisional method as the advantage of thread lifting, and is receiving a great response from a new consumer group. Without making incision, it uses a tool that can control its depth to insert special elastic band under the skin down to SMAS layer. The elastic band that is used for the procedure has a structure similar to ligament tissues in human body, so there is no feeling of irritation. The thread also has elasticity, different from the ones used in general, and it can be moved along with the facial muscle, enabling us to make natural facial expressions.

What are the expectations for non-incisional SMAS lifting?

At ID Hospital we have conducted a survey to patients who had non-incisional SMAS lifting, and 85% of them said they chose it for natural surgical result, and 75% of them said for short recovery period. (random sample: 100 patients, 2 questions multiple answers) They preferred procedure that does not affect work life but has clear surgical effect to surgery. Orthognathic surgery vs Facial contouring surgery for a smaller face, which would be the best one for me?

Ji Young Kim, aged 23, has made a huge decision. She grew up being bullied and called “Big rock face” for her large face since she was little. She visited a plastic surgery hospital to change her protruding cheekbone and asymmetrical face into a smaller and slimmer one. However, after her consultation she couldn’t decide which one to choose, orthognathic surgery or facial contouring surgery.

Similar to her case, a lot of people choose orthognathic surgery or facial contouring surgery, expecting a grand and noticeable change of their facial size. Once you get an orthognathic surgery, you can improve your mastication function including chewing food and speaking, and since the location and shape of the jaws are changed to normal, you can see a dramatic facial change.

Facial contouring surgery, on the other hand, is for changing the shape of your facial bone to make your facial shape prettier, and it includes square jaw surgery, cheekbone reduction surgery, and chin surgery. In case of strong and rough facial appearance due to too much growth of facial bone, the surgery reduces the size of the bone and changes the facial shape into a smoother one, so it is focused more on making aesthetical improvement.

Whether you should get orthognathic surgery or facial contouring surgery is decided based on mastication of your teeth, shape of the jaw bone, and the condition of the central line of your face. If you have malocclusion, jaw bones are not in the right place, or the central line of your face is tilted, you should get orthognathic surgery that corrects the placement of upper and lower jaw as well as their shape. However, if the central line of your face is straight, and you’re simply worried about the shape and appearance of your lower jaw bone, getting orthognathic surgery can solve the problem of your facial shape.
